Community Habilitation
The Community Habilitation Program provides one-to-one individual, customized support and services to Traumatic Brain Injury Survivors and individuals with Intellectual Disabilities at the location in which they reside. This service takes place in the home, whether they live independently or with family members.
The goal of this program is to develop life management skills as well as community integration. The supports provided are based on individual needs and are person-centered. These services include, but are not limited to household management, financial management, nutritional training, shopping, recreation and self advocacy. The frequency and duration of these services are specific to each individual’s needs. In addition, agency staff provide transportation for the individual into the community.
